- Proof of Veteran Eligibility ( DD214, VA card)
- Proof of EP County residency ( utility bill)
- Marriage Certificate ( for dependents and surviving spouses)
- Birth Certificate ( for dependents ages 21 and under)
- Death Certificate ( for surviving spouses).
